Transgender Awareness Week, Evaluation

A half-sheet of paper, front and back, offered to attendees of 2007 Tucson Transgender Awareness Week events. It asks whether the attendee is part of the university, where they live, for "relevant identities" (with an open-answer area for response), a question about how the attendee heard about the event, feedback about the event itself. The evaluations were created by University of Arizona's Social Justice Leadership Center and is heavily focused on student attendees, asking their years at the UofA, their course of study, whether they live on-campus or off-campus, etc.

Used for November 2007, but the evaluation form seems generalized, so it's possible it was made prior to the event for all Social Justice Leadership Events.
